Service Description
School Liaison Coach: Consolidated Service List (11 Bullet Points) Monitor academic progress including grades, assignments, attendance, and engagement, while coordinating with teachers for needed supports. Attend key school events such as Back-to-School Night, Parent–Teacher Conferences, IEP/504 meetings, and reentry planning sessions. Provide consistent mentoring and coaching focused on accountability, emotional regulation, conflict resolution, and positive decision-making. Engage families regularly to strengthen school–home communication and connect them with community resources and supports. Coordinate wraparound services by serving as the bridge between schools, probation officers, counselors, and community agencies. Support behavior intervention and restorative practices through early intervention, mediation, and harm-repair processes. Guide career and future planning including job readiness, CTE connections, internships, and exploration of postsecondary pathways. Teach essential life skills such as organization, time management, communication, budgeting, and digital literacy. Provide trauma-informed support by identifying triggers, connecting youth to counseling, and advocating for trauma-sensitive practices. Track and report progress through case notes, data monitoring, and regular updates to school and community partners. Facilitate reentry and transition planning to ensure smooth movement back into school, programs, or grade levels with clear goals and supports.
